We welcome new patients who live within our practice area. The reception team will advise new patients if their home is within our area. All new patients are asked to complete a registration form and health questionnaire prior to registration at the practice. These are available from our receptionists. The practice will not discriminate against any patient wishing to join our practice list on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.
New patients are able to book a GP or Nurse appointment on the day following receipt of their registration forms. In certain circumstances an emergency appointment can be made.
Patients on repeat medications will need to see a doctor before getting their first prescription. An appointment will be offered as appropriate.
PLEASE NOTE YOU WILL NEED TO COME INTO THE SURGERY TO SIGN AND FILL IN FORMS TO BE ABLE TO REGISTER WITH THIS PRACTICE. THANK YOU.
Unfortunately, we no longer offer on-line registration. If you wish to register with our practice please call into the surgery and fill out the appropriate forms.
