The Learning Disabilities Mortality Review (LeDeR) Programme aims to improve services and reduce health inequalities for people with learning disabilities through local reviews of deaths.
Since June 2017, our local Steering Group has received 43 death notifications and our LeDeR reviews have identified many examples of good practice.
Anyone, including people with learning disabilities themselves, family members, friends and paid staff can make a notification via this link http://www.bristol.ac.uk/sps/leder/notify-a-death/

Please share these links and make a report if you become aware of anyone with a learning disability who has died.
Multiple notifications of the same death are no problem as each case is assigned a unique number to avoid duplication.
